(1) Three Copies of Your Request For Live Scan Form from the requesting agency:
The requesting agency should furnish you with the Request For Live Scan form. The form must list the ORI number and the official title of the applicant position.
Please insure that your Live Scan Form is filled out and completed with all the required information
before you arrive for your appointment. Our technicians are not allowed to assist or complete the form for you. If the information is incomplete or you have questions, we must refer you back to the requesting agency.
There is an additional charge of $.50 cents per copy made at our facility.
(2) Two forms of ID, including a current photo ID From the list below:
California Driver’s License
California DMV ID Card
Out of State Driver’s License
Passport
Alien Registration / Immigration Green Card
Military ID Card
Employment Authorization (Form #688B), Issued by DHS With Photo
Permanent Resident Card (#151 or #551) With Photo
Expired identification information cannot be accepted. No Exceptions.
(3) Cash, Cashier Check or Money Order
The total cost of the Live Scan process involves 2 fees; the background investigation fee and the rolling fee. The background investigation fee varies according to the type of application and the level of investigation (DOJ/FBI) required by the applicant agency.
The rolling fee is a flat $20 for all applicants.
We do not accept debit or credit cards.
