At AngeliCare, we believe choosing and maintaining a care plan doesn’t have to be difficult, time-consuming, or stressful. We’ve streamlined the in-home care system to meet the needs of mature adults with these five simple steps:
STEP 1: Preliminary Acquaintance
When you first contact AngeliCare, we’ll start by building a profile of the home care recipient. We’ll ask you some basic questions regarding their:

Based on this information, we can determine whether our services can meet their needs. If so, we will schedule an in-home assessment.
For those whose needs aren’t compatible with our service, we can provide alternative resource information.
STEP 2: In-Home Assessment
The assessment is an in-depth, fact gathering consultation that is the foundation in creating a customized home care plan. One of our Care
Specialists will visit the senior’s residence and gather the necessary facts to facilitate the process. Below are key topics that will be covered during the assessment.
STEP 2: In-Home Assessment
The assessment usually takes between 1-2 hours. We encourage the participation of all family members, friends, and anyone else involved in the care of the client. Their input not only helps us to better understand the interpersonal dynamics and specifics involved, but also allows family and friends to communicate questions, concerns, or suggestions related to the care process.

Assessments are required for all services
Assessments are required in order to ensure the best level of care. You can schedule an assessment up to six months prior to beginning an in-home care program, giving you plenty of time to prepare for starting service. After six months, the assessment will need to be performed again before in-home care service can be scheduled.
Step 3: Customized Care Plan
After the assessment, the gathered information is used to create a Customized Care Plan. Each unique plan details the scheduling of our CareGivers and the level of services required to meet the client’s needs. A detailed listing of home care options and services can be found on our

A copy of the Care Plan will remain in the home of the care recipient. All related information is documented and stored, and can be revised by everyone involved in the home care process. Authorized family members can view details of the client’s care, routines, health status, and daily activities.
STEP 4: Selection of Care Providers
The selection of Care Providers is the key decision in our Care System. This is why the in-home assessment is so important, because not only does it tell us of your specific care needs, it provides the basis in the selection of the caregivers. Using a blend of innate perception, experience and specialized software, we specifically partner clients with like-minded caregivers to foster trust and rewarding relationships.
In the rare event that a match isn’t successful our management team will immediately work to find a suitable replacement. We believe that open
communication is essential, and we encourage our clients and their families to convey all questions and concerns directly to our management
team.
Our CareGivers are experienced professionals who understand that reassignment is a routine part of the home care process. We’ll continue the placement process until the care recipient, the family members, and the CareGiver are all completely satisfied.
STEP 5: Continual Care Management
To meet our client’s constantly changing needs, we strive to make our home care services as flexible and versatile as possible. Our CareGivers are fully trained to monitor and document all aspects regarding their client’s health, and report any concerns to all responsible parties. Daily documentation is always available for review in the Care Plan Book, and can also be transmitted electronically as needed.
If we determine that a client's care needs have surpassed our service level, we’ll immediately notify the family of the need for a change in coverage. Through this proactive approach, you can rest assured that you or your loved one will receive the appropriate, qualified level of home care.
